Foster-to-Adopt Application

Fostering-to-adopt is a great way to see if a dog is right for you before committing to adoption. WHAT IF THE DOG ISN'T RIGHT FOR ME? That's okay! We do count on our "foster to adopt" families to provide a short term home for our rescue dogs even if they decide not to adopt. Unless it's a emergency, we ask you give us some time to find your foster-to-adopt dog a new place to stay.

The Foster-to-Adopt Program was created to place animals into approved forever homes in a foster situation prior to adoption. The Foster-to-Adopt Program allows approved applicants to take an animal into their home to monitor the dynamic with other pets and make sure the addition to the family is a positive experience for everybody involved. In order to be accepted into this program, Foster-to-Adopt applicants must have an approved application and home visit with Addicus Legacy Dog Rescue.
You must be over 21 years of age to foster-to-adopt or adopt from Addicus Legacy Dog Rescue.

1. ALDR is entering into this Foster-to-Adopt Agreement as a courtesy to me to allow me to have custody of this animal for two weeks when a final decision will be made and I may or may not finalize the adoption process.

2. I must comply with ALDR policies concerning this animal while he/she is still in temporary foster care.

I understand that the custody of this animal will be temporary until the adoption process is complete.

As a Temporary Foster Home for ALDR, with the intention of adopting the fostered animal, I agree to comply with the following conditions and requirements:

ALDR Foster do Adopt Contract 

1. Should I decide to adopt this cat/dog, I agree to pay an adoption fee between $200 and $400 - to be determined by ALDR - at the time of signing the final adoption contract.

2. The animal is and shall remain the sole property of the ALDR untill the adoption contract is signed and adoption fee is received by ALDR. If for any reason the adoption is not finalized, I understand that the animal(s) shall be returned to ALDR.

3. I will provide the animal with good care, including but not limited to, proper food, fresh water, shelter, exercise, grooming, training and medication when required.

4. I will comply with all instructions received from ALDR and will not deviate from any instructions as to the care and maintenance of the dog without consulting with a representative of ALDR.

5. I agree to maintain the animal(s) as my own household pet(s) and companion(s).

6. I agree to comply with all TX and CT State laws and local ordinances applicable to the animal(s).

7. I agree to keep the animal(s) under my control and not let dogs roam freely outside of home or fenced yard. Keeping a dog on a chain, rope, dog-run or similar device is not permitted.

8. I will provide a safe and loving home environment and will socialize the animal to humans and other animals. Dogs are not to be excessively crated (not more than a total of 4 hours in a 24 hour period).

9. I will provide basic training or reinforcement of proper house manners and obedience skills. This training will be done only by positive reinforcement. At no time will the foster to adopt home use punishment as a training tool.

10. I agree to properly supervise said dog at all times. I will be physically present when a dog is allowed outside during which time the dog will either be in a fenced area and/or leashed.

11. I agree to the following in regard to veterinary care:
a. I will be provided with veterinary records that show that the animal has received appropriate veterinary care to date and is in good health or if there are any exceptions that I should be made aware of.
b. If the dog needs medical attention, a member of ALDR must be contacted immediately and must approve all vet visits

12. This agreement is not transferable. If I am unable to care for the animal I will return it to ALDR.

13. I understand that other animals in my household could be exposed to medical or behavioral conditions that have not been recognized in the foster animal placed with me by ALDR, and ALDR is not liable for any disease or injury of my own companion animals or other exposed animal.

14. I agree not to attempt to hold ALDR responsible for any damages which the animal may do to any person or property. I specifically release ALDR of any responsibility pertaining to any damage to property or personal injury or any occurrence relating to the dog and shall hold ALDR harmless regarding any damage or injury(ies) of any nature whatsoever, and agree to notify ALDR immediately of any damages and/or injury(ies). I understand that if I am approved for fostering, this declaration represents a legal contract between me as the foster home caregiver and Addicus Legacy Dog Rescue. I understand that if I am approved to foster an animal, I must abide by this agreement and that this agreement applies to any and all animals that I foster with Addicus Legacy Dog Rescue Accuracy of Information By signing below, I acknowledge that the information provided on this application is correct to the best of my knowledge. If at any time the information I have provided changes, I will provide the updated information to the Addicus Legacy Dog Rescue Foster Care Program.
